The Riverside County Office of Vital Records only holds death certificates in which death occurred within the county of Riverside during the current year or the previous year.
To request a certified copy of a still birth or fetal death certificate by mail we will require the following information:
- The decedent's full name
- The date of death
- The place of death
- The name of the mortuary or cremation society that handled the services
- The mailing address of where we are to forward the certified copies
The fee for a Certificate of Still Birth: $20.00
The fee for a Certificate of Fetal Death: $18.00
• Only the mother or father is eligible to request a certified Certificate of Still Birth.
• Complete the appropriate application for the type of record you are requesting (Still Birth or Fetal Death) - be sure to complete all items on the application.
We accept personal checks and money orders. If paying by personal check, the check must be pre-printed with check holder's information such as name and complete physical address. No alterations or white out are accepted.

The application and sworn statement can be printed from our Web site. If you do not have access to a printer you can contact us at (951) 358-5068 or e-mail us to have an application mailed to you.
Once you have completed the application and the sworn statement has been notarized, please mail your request with the appropriate fees to:
